CBD Stone Removal
CBD Stone Removal (Common Bile Duct Stone Removal) is an advanced endoscopic treatment used to remove gallstones that have moved from the gallbladder into the common bile duct. These stones, known as common bile duct stones or choledocholithiasis, can block the normal flow of bile and may lead to pain, jaundice, infection, pancreatitis, and other complications.
Common bile duct stones may be discovered during an evaluation for abdominal pain, abnormal liver function tests, jaundice, or imaging abnormalities. In some patients, they are identified even when there are few or no symptoms. When a stone is causing obstruction or other complications, timely treatment is important.
Our CBD stone removal service provides advanced endoscopic evaluation and treatment, most commonly using Endoscopic Retrograde Cholangiopancreatography (ERCP). The procedure allows the specialist to access the bile duct, remove stones, and restore normal bile drainage without requiring an open surgical procedure.
What Are CBD Stones?
The common bile duct (CBD) is a tube that carries bile from the liver and gallbladder into the small intestine. Bile helps the body digest and absorb fats.
Gallstones form most commonly in the gallbladder. Sometimes, a stone can move from the gallbladder into the common bile duct. Once inside the duct, it may become lodged and obstruct the normal flow of bile.
CBD stones can vary considerably in size and number. Some are small and may pass naturally into the intestine, while others remain trapped in the bile duct and require medical treatment.
Symptoms of Common Bile Duct Stones
The symptoms of CBD stones can vary depending on whether the stone is causing complete or partial obstruction.
Common symptoms may include:
- Pain in the upper abdomen
- Pain that may radiate toward the back or right shoulder
- Nausea or vomiting
- Yellowing of the skin or eyes (jaundice)
- Dark urine
- Pale or clay-coloured stools
- Itching
- Fever or chills when infection develops
Some patients may have abnormal liver blood tests without significant symptoms.
A combination of fever, jaundice, and abdominal pain can indicate a potentially serious bile duct infection known as cholangitis and requires prompt medical evaluation.
How Are CBD Stones Diagnosed?
Diagnosis generally begins with a clinical assessment and blood tests. Liver function tests may show abnormalities when a stone obstructs bile flow.
Imaging studies are commonly used to identify stones or evidence of bile duct obstruction. These may include abdominal ultrasound, CT scans, or magnetic resonance cholangiopancreatography (MRCP).
In selected cases, endoscopic ultrasound (EUS) may be used to identify small bile duct stones that are difficult to detect with other imaging techniques.
ERCP is generally used primarily as a therapeutic procedure when stone removal is required rather than simply as a diagnostic test.
ERCP for CBD Stone Removal
ERCP is one of the most commonly used minimally invasive treatments for common bile duct stones.
During ERCP, a flexible endoscope is passed through the mouth into the small intestine. The opening of the bile duct is identified and carefully accessed.
Contrast dye may be injected into the bile duct while X-ray imaging is used to visualize the duct and identify stones or other abnormalities.
Once the stones have been located, the specialist can use specialized instruments to remove them.
How Are CBD Stones Removed?
A common technique involves making a small opening at the bile duct outlet, known as a sphincterotomy, when appropriate.
A retrieval balloon or basket can then be used to capture and remove stones from the common bile duct.
Smaller stones can often be removed directly. Larger, impacted, or difficult stones may require additional techniques to make removal safer and more effective.
Depending on the size and characteristics of the stone, options may include mechanical lithotripsy, cholangioscopy-guided intraductal lithotripsy, or other advanced stone-fragmentation techniques.
If complete removal is not possible during the initial procedure, a temporary biliary stent may sometimes be placed to maintain bile drainage while further treatment is planned.
Treatment of Large or Difficult CBD Stones
Some bile duct stones are considered difficult because they are very large, impacted, numerous, or located in a challenging part of the bile duct.
In such cases, standard ERCP techniques may not be sufficient.
Advanced endoscopic methods can be used to fragment the stone into smaller pieces. Cholangioscopy-guided lithotripsy allows the specialist to directly visualize the stone inside the bile duct and use a specialized energy device to break it into smaller fragments that can subsequently be removed.
The most appropriate approach depends on the patient’s anatomy, stone characteristics, available expertise, and overall medical condition.
What to Expect During CBD Stone Removal
CBD stone removal is generally performed in a specialized endoscopy unit under sedation or anesthesia.
Before the procedure, your specialist will review your symptoms, medical history, blood tests, imaging, medications, allergies, and any previous gallbladder or bile duct procedures.
You will usually be asked to fast before ERCP. Special instructions may be given regarding blood-thinning medications and other medicines.
During the procedure, the endoscope is passed through the mouth and into the small intestine. The bile duct is accessed and evaluated using contrast imaging.
The stone or stones are then removed using appropriate endoscopic instruments. A biliary stent may be placed if necessary to ensure adequate drainage.
After the procedure, you will be monitored for a period before discharge or hospital admission, depending on your condition and the complexity of treatment.
Benefits of Endoscopic CBD Stone Removal
For appropriately selected patients, ERCP-based stone removal offers several potential benefits:
- Minimally invasive treatment
- No large surgical incision
- Direct access to the common bile duct
- Effective removal of many bile duct stones
- Restoration of normal bile drainage
- Ability to treat associated bile duct obstruction
- Advanced options for large or difficult stones
- Potentially shorter recovery compared with more invasive approaches
When gallstones remain in the gallbladder, additional treatment may be recommended to reduce the risk of future stone-related problems.
Risks and Possible Complications
Although ERCP is a well-established treatment, it is an advanced procedure with potential risks.
Possible complications include post-ERCP pancreatitis, bleeding, infection or cholangitis, perforation, abdominal pain, and adverse reactions to sedation or anesthesia.
The risk varies according to the patient’s medical condition, anatomy, complexity of the stones, and type of intervention required.
Your specialist will explain the potential benefits, risks, alternative treatment options, and follow-up plan before the procedure.
After CBD Stone Removal
Following successful stone removal, your doctor may recommend additional evaluation of the gallbladder if it remains present. In patients with gallstones in the gallbladder, removal of the gallbladder may be considered to reduce the likelihood of recurrent stone-related complications.
Follow-up blood tests or imaging may be recommended depending on the patient’s symptoms and initial findings.
You should seek urgent medical attention if you develop severe abdominal pain, persistent vomiting, fever, chills, worsening jaundice, or other concerning symptoms following the procedure.
